Cascade Terrace is located in SE Portland on 122nd Ave.  It is easily accessible from I-205 and is just north of Foster Road.  It is a 105 bed nursing facility that provides skilled and long term nursing services.  The facility boasts an active rehab and transitional care program.   Cascade Terrace is a wonderful solution for patients who are stable and no longer require hospitalization but who need an intensive program of rehabilitation and recovery.  The care team works 24 hours a day to help many patients resume their former lifestyles and to assist others in becoming as independent as possible.

Cascade Terrace was built in the 1980’s and offers bright, cheerful lounge and dining areas.   The room selection includes private, semi-private and 3 bedrooms.  The facility’s transitional care unit includes an occupational therapy kitchen for home skill training.   Cascade Terrace is a contracted provider for Medicaid, Medicare, and several managed care health plans.
