Chehalem Health & Rehab is located on Fulton St. just blocks from the local hospital.  The facility is tucked away on a quiet residential street near a city park.  Chehalem is a 99 bed nursing facility that provides traditional nursing and rehabilitative care.  Assistance and support is provided by a team of care givers whose goal is to become a positive part of the residents’ daily lives – helping patients return home or assisting others in becoming as independent as possible.  The facility also offers a specialized Alzheimer’s and dementia care unit.  The unit’s atmosphere is designed not to overwhelm the residents but to add structure and familiarity to their lives.  Each resident is unique in their needs and the staff takes great care to communicate and learn the activities and memories that add comfort to their day.

Chehalem Health & Rehab joined the Prestige family of facilities in August 1998.  The room selection includes private, semi-private and 3 bedrooms.  Chehalem is a contracted provider for Medicaid, Medicare, and several managed care health plans.
