Park Forest Care Center is located in a quiet NE Portland.  Despite its urban location, its backyard is the peaceful forested area of The Grotto.  This 55-bed nursing facility provides skilled and long term nursing services.  Its skilled care program is a wonderful solution for patients who are stable and no longer require hospitalization but who need an intensive program of rehabilitation and recovery.  Assistance and support is provided by a team of care givers whose goal is to become a positive part of the residents’ daily lives – helping patients return home or assisting others in becoming as independent as possible.  The setting is also ideal for residents who need assistance with routine personal care activities and health monitoring by licensed nurses.  Residents of Park Forest receive a very personal approach to their care.

The restful peace and the serenity of nature are readily apparent in the gardens, walk-ways, flowers, and trees at Park Forest.  Special activity areas are included for those who wish to participate in outdoor games.  Raised gardens for flowers and vegetables are available for those in wheelchairs.  The room selection includes private, semi-private and 3 bedrooms.  Park Forest is a contracted provider for Medicaid, Medicare, and several managed care health plans.
